2. Final Forms Registration
Lakeland Prep requires all athletes to be registered with Final Forms
3(a). USA Lacrosse (FOR MS PLAYERS ONLY)
All new and returning MS players are required to register/ renew with USA Lacrosse. TSSAA insurance covers HS players. A membership is not required for HS players. Once completed, be sure to complete step 3(b). Be sure to email Coach Cook with any questions: scook@lakelandk12.org

3. Code of Conduct Waiver (Required only for HS Team)
This is a code of conduct form to be signed by parents regarding what is expected of them in the stands.
4. Register for The 2026 Season
***As a returning player, you may skip this step*** In order for the SE App to operate properly complete this step last once the prior three have been done. Once finished go to the home page and select "Home," "App Setup" from the pulldown menu. I will do some behind the scenes work and once I am done, you will have full SportsEngine (SE) app access.
5. Pay team fees on Skyward or to the school directly
The total fee for the season is $400 which covers things like referees, field striping, uniforms and player gear. To avoid the 3% processing fee on Skyward a check can be delivered to the school.
6. Media Release
Required in order for your son's image to be used
7. Uniform Contract
All players are required to fill out the attached document in order to receive a uniform
